If you cannot complete your registration and medical history online you may print and complete these forms before coming in for your appointment. If any patient information has changed since your last visit (i.e. insurance carrier, address, etc.) please update your information online, or print and complete a new Patient Registration Form.

To authorize to send or obtain medical records, please select the appropriate link below. (When filling out the authorization – be sure to initial by the records you choose to send or receive.)

Steps For New Patients

The following steps will help you see the doctors and other care professionals at St. Joseph’s ENT Clinic

  • Do You Need A Referral?

    Some insurance plans require that you get a referral to our clinic, while others will let you make an appointment without one.

    1. Call your insurance.
    2. See your general practitioner or if you’re allowed to make an appointment without a referral, call St. Joseph’s ENT Clinic
    3. Ask to see either Dr. Thomas deTar or Dr. Erik Gilbert
    4. Choose the best date and time that works for you
  • Make An Appointment

    Call our office at (208) 777-1320 and make an appointment. Make sure to write down your medical history security token.

    • Medical history security token – this is an 8 digit code (in the form of xxx-xx-xxx) which you will use to fill out your medical history online
  • Complete Your Medical History

    Create an account with NextGen and complete your medical history. You will need your security code to create your account. If you do not have this code, please call our office at (208) 777-1320.

  • Arrive Safely For Your Appointment
